When the medicine is not available

Why shortages hit small markets first, what a pharmacy can do without a prescriber, what only a doctor can authorise, and where each agency publishes its notified supply problems.

Why they happen, and why here

Modern medicine manufacturing is concentrated: a single plant can supply a large share of the world's stock of an active ingredient, and a quality problem at that plant becomes a global shortage within weeks. Add demand shocks, currency and pricing effects that make one national market less attractive than another, parallel export from lower-priced markets to higher-priced ones, and the ordinary fragility of long supply chains. Then add the specific problem of this region: Estonia, Latvia, Lithuania and to a lesser extent Finland, Norway and Denmark are small markets by volume, so when supply is allocated they are not the priority. A medicine that is merely inconvenient to obtain in Germany can be genuinely unavailable in Tartu.

What the pharmacy can do on the spot

More than people expect. It can check whether stock exists at another branch or another pharmacy in the town. It can order for the next delivery, which in this region is usually the next working day. It can dispense an interchangeable equivalent from a different manufacturer under the ordinary substitution rules. It can propose a different strength or pack that makes the same daily dose — two of a half-strength tablet instead of one of the whole, where the formulation allows — but that needs the prescriber's agreement in many systems and always needs the arithmetic to be right and written down. And it can tell you whether this is a national shortage or a local stock gap, which changes the entire plan.

What only a prescriber can do

Change you to a different medicine. Authorise the use of an unlicensed product or an import where a national exemption route exists. Alter a dose or a formulation where that is not a straight equivalence. If the shortage is national and prolonged, this is the conversation to have early rather than at the end of the last pack, because those routes take time. Never solve a shortage by taking someone else's prescription, by adjusting your own dose to make the supply last, or by buying the missing item from an unregistered online seller — the third of those is the exact market that falsified medicines are sold into.

The shortage registers

Shortages are not a rumour: they are notified. Under the EU medicines code, the holder of a marketing authorisation must notify the competent authority in advance where it will interrupt supply of a medicine, and every national agency in this region publishes a list of the supply problems it has been told about. Those lists are the authoritative answer to "is this a real shortage or just this pharmacy", and they usually say whether an alternative strength or an alternative product is available. Your pharmacist reads them; you can too. Each country page in this surface names the agency that publishes its list.

Planning around one

If a medicine you depend on is on a shortage list, three things help. Order the repeat earlier than usual, within what your prescription and your reimbursement rules allow — remember that Finland reimburses at most three months at a time and other countries have their own caps, so this is timing rather than stockpiling. Ask the pharmacy to note you as a regular user of that item, because a pharmacy that knows a named patient depends on something allocates it differently from anonymous shelf stock. And ask the prescriber, once, whether there is a second-choice product they would be content with, so that the decision is already made when the gap arrives.

What not to conclude

A shortage of one brand is not a shortage of the medicine — in most cases another manufacturer's equivalent is on the shelf and the substitution rules cover it, which is why the box changed. A pharmacy that says "we cannot get it" may mean today, this week, or nationally, and those are three different problems; ask which. And a shortage is not a reason to stop a treatment silently. If the gap is going to be real, that is information the prescriber needs while there is still time to plan, not afterwards.

Dažnai užduodami klausimai

Is there an official list of medicine shortages?

Yes. Marketing authorisation holders are legally required to notify the national agency in advance of a supply interruption, and each agency in this region publishes the notified shortages. That list is the authoritative answer to whether the problem is national or local to one pharmacy.

Why are shortages worse in the Baltic countries?

Because allocation follows volume. Estonia, Latvia and Lithuania are small markets, so when global supply is constrained they are not the priority, and parallel export towards higher-priced markets makes it worse. A medicine that is inconvenient elsewhere can be genuinely unavailable here.

Can the pharmacy give me a different strength?

Sometimes, where the formulation allows the same daily dose to be made up differently. In many systems that needs the prescriber's agreement, and it always needs the arithmetic written down. Ask rather than improvising at home.

What should I not do in a shortage?

Do not stretch your supply by changing your own dose, do not take someone else's prescription, and do not buy the missing item from an unregistered online seller — that is precisely the market falsified medicines are sold into.
