Returning unused and expired medicines
Why the bin and the toilet are the wrong answers, what pharmacies take back, how to handle sharps, and the annual clear-out that prevents most household medicine errors.

Why not the bin or the toilet
Two different problems. Flushed medicines go through a wastewater treatment plant that was designed to remove organic load and nutrients, not pharmaceutical molecules, and a substantial fraction passes into rivers and the sea — this is measurable and measured across the Baltic catchment. Medicines in ordinary household waste end up in landfill or incineration, and in landfill they leach; before that, they sit in an unsecured bin bag where a child or an animal can reach them. The pharmacy route exists in every country in this region, is free for households, and takes the same trip you were making anyway to collect a repeat.
What a pharmacy will take
Unused and expired human medicines from a household: tablets, capsules, liquids, creams, inhalers, patches, eye drops. Practice varies on the details — whether to keep them in their packaging, how liquids should be presented, whether inhalers and cytotoxic medicines go through a separate route — so ask once and then do it their way. Some countries and municipalities also run collection points outside pharmacies. What is generally not part of the pharmacy route is veterinary medicine in bulk, chemicals that are not medicines, and clinical waste from a professional setting; those have their own municipal arrangements.
Sharps
Needles, lancets and pen tips never go into a bag, a bottle or household waste. They go into a rigid, puncture-proof container that closes — pharmacies sell them, and in some countries supply them with the prescription for the device. When it is full, it goes back to the pharmacy or to the municipal collection point that handles it in your country. This matters for the people who empty bins as much as for you. If you have been using an improvised container, ask the pharmacy what they will accept; the answer is usually pragmatic, and the alternative is worse.
Take the name off first
A dispensing label carries your name, often your address, and the medicine you take. That is health data on the outside of a box you are handing to a stranger and then to a waste process. Peel it off, cover it, or score through it before the pack leaves the house. This is the step nobody does and the one with the clearest privacy benefit — and for anyone clearing a house after a death, it is worth doing systematically before the trip.
Why the clear-out matters more than the disposal
The environmental argument is real, but the household argument is stronger. Expired and superseded packs in a cupboard are the raw material of every domestic medicine error: the old-strength box next to the new one, the substituted generic beside the original with the same active substance in it, the antibiotic saved from last winter and taken for something it does not treat, the medicine somebody stopped taking two years ago that a visitor helps themselves to. Clearing them out is a safety intervention, not a tidying exercise — see why the box changed for the specific double-dosing failure it prevents.
Make it a habit
Once a year, and again after any hospital discharge or change of treatment, put everything on a table: check the expiry dates, check whether anything is a duplicate active substance under two names, and set aside anything nobody is taking. Take the discard pile to the pharmacy on the next repeat trip, and take the keep pile with you too if a medication review is available — a pharmacist looking at the actual boxes will find things a list does not show. What else a pharmacy does covers the review; keeping medicines at home covers where the keep pile should live.

Частые вопросы
Where do I take old medicines?
To a pharmacy. Every country in this region operates a household take-back route through pharmacies, free of charge. Some municipalities run additional collection points. Do not put them in household waste and do not flush them.
Can I flush medicines down the toilet?
No. Wastewater treatment is not designed to remove pharmaceutical molecules and a substantial fraction passes into surface water. The pharmacy take-back route exists precisely so that this is not necessary.
What do I do with used needles?
They go in a rigid, puncture-proof container that closes — never a bag or a bottle. Pharmacies sell them and in some countries supply them with the device. When full, it returns to the pharmacy or the municipal point that handles sharps in your country.
Should I remove the label before returning medicines?
Yes. The dispensing label carries your name and what you take. Peel it off or score through it before the pack leaves the house — the pack is going to pass through several hands after yours.
